Add's Global Custom fields to Open Ticket / Send Mail so they can be set when creating new tickets.

Patch based off Chris Browning work. I simple added it to both Open Ticket / Send Mail instead of Just Open Ticket Here is a link to his github patch. http://github.com/stolf/cerb4/tree/jira/chd-1278
